Patient-Centered Medical Home (PCMH)

NCQA-PCMH-Bagde

The Patient-Centered Medical Home (PCMH) is a care model that focuses on putting patients first. It improves the relationship between patients and healthcare teams, leading to better quality care, enhanced patient and staff satisfaction, and lower healthcare costs. Practices that follow this model commit to ongoing quality improvements. The PCMH Recognition program by NCQA is the most commonly used to evaluate these practices.

Why PCMH?

The PCMH model emphasizes team-based care, communication and coordination, which has been shown to lead to better care.

Higher rates of fragmentation are associated with higher costs, lower quality and higher rates of preventable hospitalizations.
